Support Planner

Job Type: Therapy Services (Allied Health)
Hours: Permanent Full-Time
Location: Townsville, Yeerongpilly, QLD
Pay: $37.35 p/h + super + salary packaging

About The Role

The Support Planner plays a key role in coordinating and supporting service access for individuals and families engaged with Deaf Connect’s Therapy and Family Services. This role involves working collaboratively with clients of all ages, their families or carers, clinicians, and internal teams to understand individual needs, identify goals, and facilitate appropriate supports and service pathways. The Support Planner ensures that engagement and planning are person-centered, culturally safe, and responsive to the diverse needs of the Deaf and hard-of-hearing community. 

This position also acts as a central communication point between clients, families, internal service teams, and relevant external stakeholders, supporting continuity of care and coordination across programs and systems. 

Main responsibilities of the role include: 

  • Work closely with participants and their families to create personalised allied health support plans using NDIS funding. 
  • Coordinate with allied health staff to check their availability and organise service timetables  
  • Prepare detailed Service Agreements and make sure service bookings are entered correctly. 
  • Work with NDIS partners (like ECEI, LACs), Plan Managers, and Support Coordinators to make sure participants get the support they need. 
  • Help participants and families get ready for NDIS plan reviews. 
  • Provide quotes for services and help sort out any unpaid invoices. 
  • Manage the waitlist for all allied health services (e.g. OT, physio, psychology, and Teachers of the Deaf). 
  • Organise internal referrals and keep track of any appointment cancellations. 
  • Work with the Admin Assistant to make sure Service Agreements are properly recorded and that support is delivered as planned. 
  • Pass on important information to the relevant leadership team (e.g. Clinical Manager, Family Services Lead, Team Leads, or General Manager) to support smooth service delivery. 
  • Keep accurate records and provide monthly reports to the General Manager, including details on referrals, new clients, plan renewals, discharges, and other key data. 

About You

  • Excellent communication and interpersonal skills, with the ability to engage effectively with a range of internal and external stakeholders. 
  • Strong attention to detail and well-developed organisational skills, including accurate documentation, data entry, and effective time management. 
  • Understanding of the needs and experiences of Deaf and hard of hearing children and their families. 
  • Professionalism, patience, and cultural sensitivity when working with individuals and families from varied backgrounds. 
  • Current Queensland Blue Card and NDIS Worker Screening Card. 
  • Completed NDIS Worker Orientation Module. 

 

The following would be advantageous:   

  • Understanding of NDIS processes
