Support Planner – Ageing Well | South Australia

Make a positive impact in supporting our ageing well clients, as part of our multidisciplinary team to achieve quality individual service programs for our deaf and hard of hearing clients.

Deaf Connect is seeking a dedicated Support Planner to join our multidisciplinary team to support in the management of support plans for our deaf and hard of hearing ageing well clients. Providing customer-centric services and support ensuring our client’s needs and preferences are met while promoting independence and well-being.  

Reporting to the General Manager Ageing Well, your role will be responsible for carrying out support planning activities in a timely manner as well as ensuring a high-quality service provision that meets compliance with all relevant department standards and regulations. 

Additional responsibilities of the role include: 

• Manage operational aspects of funding programs  

• Maintain client information, case records, prepare reports and ensure performance reporting and data entry is maintained accurately to ensure casework accountability 

• Support and guide team members to access and understand the relevant parts of Deaf Connect quality management system 

• Assist the General Manager Ageing Well to develop budgets and allocate resources to ensure efficient and effective delivery of services
