Deaf Connect is seeking a dedicated Support Planner to join our multidisciplinary team to support in the management of support plans for our deaf and hard of hearing ageing well clients. Providing customer-centric services and support ensuring our client’s needs and preferences are met while promoting independence and well-being.
Reporting to the General Manager Ageing Well, your role will be responsible for carrying out support planning activities in a timely manner as well as ensuring a high-quality service provision that meets compliance with all relevant department standards and regulations.
